Award of Excellence & Superior Safety Application

 Eligibility

• Your company does not have to be a Utah Manufacturers Association (UMA) partner or headquartered in Utah.
• There is no entry fee required for this award.
• Self-nominations are strongly encouraged. 
• All applications must be submitted electronically through the UMA website.
• Judges and sponsors will use information in the application submitted for press releases about the award recipients.
• The award-winning companies will be notified before being announced at the MFG Safety Awards Expo+Conference during lunch on February 20, 2025.
• All award-winning companies will be allowed one attendee to the award lunch (lunch is included for all attendees).

 Award of Excellence Requirements

• Any Utah manufacturing or distribution company with an incidence rate lower than the national average for their industry can apply for the Award of Excellence.
• Your company must have established fundamental safety and health programs in place.
• Your company must have operations in Utah.
• Applications will close on January 22, 2025.

Superior Safety Award Requirements

• Companies must qualify for the Award of Excellence to be eligible for the Superior Safety Award.
• Your company must meet all of the following criteria:
  • Absence of work-related fatalities during the reporting period.
  • Improvement in safety each year over the last three years as measured by recordable cases and incidence rates or sustained levels of excellence as demonstrated by rates consistently below BLS statistics.
• Submit a narrative explaining why your company should be chosen.

Application Submission Information

• Application forms must include detailed information and will be judged based on the answers listed below.
• To submit your application form, please answer all online questions in full.
• Judges rank criteria with relevance to each applicant.
• Judges are in no way influenced by the level of membership or investment status with the association.
• IMPORTANT: The maximum file size for uploading documents or images is 2 megabytes per file. Acceptable formats are PDF, DOC, DOCX, JPG, JPEG, PNG, and GIF.
• Nominations will close on January 22, 2025.
